Ensuring a Smooth Start at North Avenue Learning Center

Review and Sign the Childcare Agreement:
Ensure you have read, understood, and signed the Childcare Agreement Form.

Complete Childcare Registration Form:
Fill out all sections of the Childcare Registration Form with your child’s personal and contact information.

Read the Parent Handbook:
Go through the Parent Handbook to familiarize yourself with our policies and procedures.

Grant Sunscreen Permission:
Sign the Sunscreen Permission form if you consent to sunscreen application on your child.

Provide Detailed Child Information:
Complete the Child Info Form with your child’s health history, dietary needs, and other important details.

Fill Out The Food Program – EIEA:
Fill out the Food Program form regardless of Income. We are participants of USDA Food Program for childern.

Sign the Photo Release Form:
Give consent for your child’s photographs to be taken and used by signing the Photo Release.

Submit Immunization Records:
Fill out your child’s immunization records with the Immunization Release Form.

Double-Check all Forms for Accuracy:
Review all filled forms to ensure all information is accurate and complete.

Submit Forms to North Avenue Learning Center:
Ensure all forms are submitted to the enrollment office by the given deadline, either electronically or in person.

What is the purpose of the Child Info Form?

How does the Food Program work?

We will provide a variety of nutrient-dense foods which are rich in whole grains, fruits, vegetables, low in salt, fat, and sugars. We will limit the amount of highly processed foods, providing healthy and delicious meals for the children always. A monthly menu will be posted in the office area for parental viewing. We follow the USDA Child Food Program. The food program requires each family to fill out an income statement annually. “In accordance with federal civil rights law and U.S. Department of Agriculture (USDA) civil rights regulations and policies, this institution is prohibited from discriminating on the basis of race, color, national origin, sex (including gender identity and sexual orientation), disability, age, or reprisal or retaliation for prior civil rights activity.”

Is a Photo Release necessary?

Yes, the Photo Release form is required for us to photograph your child during activities. These photos may be used for promotional materials or shared with parents.

What is the Immunization Release Form?

This form verifies that your child has received the necessary immunizations. It’s crucial for maintaining a healthy environment at our center.
